Feel empowered to make the necessary arrangements for care. Through Carepath’s elder care support program, you and your spouse/partner can speak with a Nurse Case Manager to help you and your family understand and work through the healthcare system.

Who’s eligible

You and your spouse/partner have access to Carepath’s elder care support program.

How it works

You and/or your spouse/partner will work one-to-one with a Nurse Case Manager, who will provide unlimited navigation assistance as your or your spouse/partner's needs evolve. That means you will have ongoing virtual nursing support, health education, and coaching to ensure you and your family have the information needed to make informed healthcare decisions.

Elder care support services

Call 1-866-883-5956 (Monday through Sunday, 7 a.m. to 11 p.m. ET) to connect with an intake specialist and, within 48 hours, you’ll be in contact with a Nurse Case Manager virtually or by phone.

Following an initial consultation and assessment with you and/or your spouse/partner, your dedicated Nurse Case Manager will develop a customized plan of care based on your concerns and preferences, and will help you and your spouse/partner with the following:

  • Preparation for upcoming medical appointments, including procedures
  • Information and services for safe and independent living, such as:
    • Discharge from hospital
    • Assistive devices
    • Medical alert providers
  • Finding the right senior living facility when staying at home is no longer safe (e.g., retirement, long-term care, and/or respite facilities)
  • Information about hospice organizations and home care support services
  • Information and access to local community resources, such as:
    • Primary care physicians
    • Physiotherapy and occupational therapy
    • Speech-language pathology
    • Dietitian services
    • Meal delivery
    • Transportation services
  • Information on local volunteer organizations that can provide additional support
  • Navigating and accessing provincially and federally funded benefits, including other insurance benefits that may be available
  • One-on-one coaching about ‘difficult conversations’ on aging care needs and life-changing moments (e.g., moving into a retirement home, long-term care facility, the loss of a spouse)
