Chronic disease support program

In the event of illness or other health crises, Carepath’s chronic disease program provides comprehensive and personalized support to you, your spouse/partner and dependants under age 26. It is an innovative healthcare navigation service that offers ongoing virtual nursing support, health education, and coaching to ensure you and your family have the information needed to make informed healthcare decisions.

You, your spouse/partner and dependants under age 26 all have access to Carepath’s chronic disease program.

How it works

Acting as partners and advocates for you, your spouse/partner and eligible dependants, Carepath’s Nurse Case Managers use their expert knowledge of the healthcare system to help you and your family understand the condition, test results, and treatments — all while offering emotional support.

Once you’re paired with a Nurse Case Manager — a specialist in the disease or condition — you, your spouse/partner and eligible dependants have a single point of contact to ensure continuity of care. The Carepath team follows the most up-to-date guidelines for all diseases to ensure the best possible outcomes.

Chronic disease support services

Call 1-866-883-5956 (Monday through Sunday, 7 a.m. to 11 p.m. ET) to connect with an intake specialist and, within 48 hours, you’ll be in contact with a Nurse Case Manager virtually or by phone.

Following a comprehensive health assessment, including a review of medical records, your dedicated Nurse Case Manager will help you, your spouse/partner and eligible dependants with the following:

  • In-depth review of treatment plans to ensure they are consistent with medical best practices
  • A plan of care based on nursing best practices
  • Explanation of diagnosis, test results, and treatment plan
  • Facilitating access to tests, treatments, and clinical trials
  • Understanding information provided by physicians and other healthcare providers
  • Education on how to manage symptoms to minimize treatment side effects
  • Information and access to local community resources, such as:
    • Physiotherapy and occupational therapy
    • Speech-language pathology
    • Dietitian services
    • Pain programs
    • Mental health support
  • Information on local volunteer organizations that can provide additional support
  • Virtual medical second opinion, when needed
  • Updating your primary care physician (with consent)
